Advanced Care Planning Improving the Quality of End-of-Life CareOur research team has developed innovative Advanced Care Planning (ACP) interventions, designed to improve end-of-life care through the seamless integration of scientific evidence into practice. Our interventions were informed by the person-centred motivational interviewing philosophy, structured by Dyadic Illness Management Theory and the Transtheoretical Model of Change, and studied through the lens of implementation science. This personalised approach aims to better understand individual patients and their values, recognising that managing their illness involves their caregivers and that feelings about the illness and ACP will move through stages over time. Our researchers evaluate the interventions through our implementation science approach. In one of our research projects, ACP implementation in 20 nursing homes in Hong Kong achieved high adoption rates among organisations and nurses, alongside significant enhanced family involvement and advance directive completion, demonstrating its feasibility and impact. Another trial targets older adults visiting emergency rooms and their family members. Preliminary results show notable improvements in emotional perception of ACP and readiness for end-of-life planning among patients and families. Our trials offered implementation tools such as pictorial ACP educational booklets for older adults and family members. Researchers also uncovered new pathways for translating ACP into routine clinical practice across settings including residential care homes, acute services in the emergency room and geriatric clinics. These projects underscore our team%u2019s dedication to translating research into effective, nurse-led ACP interventions that engage older adults, families, and healthcare professionals across diverse contexts. 45
