Healthy Ageing A Pioneering Nurse-led Response to Healthy AgeingHong Kong is projected to have the world%u2019s highest proportion of older adults by 2050. In response, our research team is committed to transforming ageing from being perceived as a burden into an opportunity using data-driven innovation and localised solutions. Our team launched the first comprehensive Healthy Ageing Registry, a groundbreaking initiative leveraging the WHO%u2019s Integrated Care for Older People (ICOPE) framework. The registry is a vital diagnostic tool, mapping the intrinsic capacity (functional reserve) of over 11,500 older adults across all 18 districts in Hong Kong. Building upon the insights provided by the Healthy Ageing Registry, our team develop the Jockey Club Pathway to Healthy Ageing Project. The programme aims to optimise the functional status of older adults using the WHO-ICOPE model, addressing the challenges faced by older adults with accelerated ageing.Supported by the Hong Kong Jockey Club Charities Trust, the Pathway to Healthy Ageing Project commenced in July 2021. Researchers worked across seven districts, collaborating with 14 District Elderly Care Centres and Neighbourhood Elderly Centres.By December 2024, approximately 2,800 older adults had participated in the 12-week Pathway to Healthy Ageing Project. Our researchers developed a personcentred case management plan incorporating various health promotion strategies for each individual. An interim analysis of the program revealed some positive effects on participants%u2019 quality of life, with improvements observed in physical mobility, balance, fall risk reduction, nutritional status, sarcopenic risk, mood, and cognitive function. We submitted programme details and preliminary results to the United Nations Decade of Healthy Ageing online platform for sharing with international stakeholders. Our research team also presented the programme impact and findings in the Healthy Ageing Symposium held at HKUMed Campus in February 2025, which included local stakeholders such as government officials, academics, and researchers. To ensure the long-term, Hong Kong-wide adoption of the Pathway to Healthy Ageing Project, our team trained 480 individuals aged 50 and above as health coaches. The comprehensive training focused on developing expertise in health assessment, health education, and lifestyle modification strategies. Following the successful impact of the health coaches, the team launched the Volunteer Captain Project for Healthy Ageing Promotion. This initiative aims to create a team of independent lay health coaches who will support the implementation of the Jockey Club Pathway to Healthy Ageing Project care model within Hong Kong%u2019s Community Care Service for the Elderly. The team is also training social workers to integrate the program into existing local services, creating robust support networks that ensure the program%u2019s continued success and sustainability. 46
