Gerontology and Long-term CareThe Gerontology and Long-Term Care research group confronts the defining challenges of our time: rapid population ageing, climate vulnerability, and the chronic disease epidemic. Using transformative, scalable innovations, the group focuses on changing approaches to care. Their work combines cuttingedge science, policy reform, and community empowerment to promote good health and wellbeing at all stages of life.Aligned with the UN Decade of Healthy Ageing, our research directly shapes Hong Kong%u2019s 2025 Elderly Services Blueprint and informs WHO guidelines. The group has earned 18 patents and seven crosssector collaborations in Asia and recognition by the International Self-Care Foundation. We are not merely studying ageing, we are designing fairer and climateresilient systems to ensure dignity and health for generations. This is gerontology redefined: proactive, precise, and profoundly human. Some highlights of the research group%u2019s work include Hong Kong%u2019s first Comprehensive Healthy Ageing Registry (15,000+ participants by 2025), a landmark initiative integrating the WHO%u2019s Integrated Care for Older People (ICOPE) framework with geospatial analytics and AI-driven risk stratification. Our transdisciplinary climate-ageing studies, in collaboration with environmental scientists and geriatricians, reveal how extreme heat exacerbates chronic conditions in older adults, informing adaptive urban policies. Our HEARTWISE study uses an AI-powered heart disease risk prediction tool to optimises triage accuracy across regional hospitals for steppedcare prescription. We also tested a theory-based care innovation to reduce heart attack care-seeking time, saving lives through behavioural nudges. Our cardiac rehabilitation programs and heart failure management models were endorsed by the European Society of Cardiology.34
